Volunteering
STEP 1-
SUBMIT ALL 4 DOCUMENTS LISTED BELOW TO THE SCHOOL OFFICE:
- Printout of the completed Online Volunteer Application
- Signed Volunteer Commitment Form
- Signed COVID Waiver & Release Form
- Physician or Clinic completed Tuberculosis Physician/Clinic Form

Adult TB Risk Assessment Questionnaire (submit with Tuberculosis Physician/Clinic Form)

Health Clearance Requirements (information on TB clearance and TB screening locations)

STEP 2-
UPON COMPLETION OF YOUR ONLINE APPLICATION
● The school office will process your application upon receipt of all required documents (1-2 days). Please check
your email for application status.
● Final approval will be completed by the LAUSD office of Parent and Community Services (PCS). You may start to volunteer when PCS emails you a temporary volunteer badge (2-4 weeks). An official volunteer badge will be
mailed to the school.
OTHER VOLUNTEER INFORMATION-

○ If you are a Tier III or IV Volunteer (e.g., overnight trip chaperone), fingerprint clearance is required.
○ After completing your application, upload your COVID-19 vaccination card to LAUSD Daily Pass, using the same email and password as you used to register for the Volunteer Management System.
○ Applications will NOT be processed by the school or District PCS office if incomplete or missing documents.
